What you are buying – and what you can choose

Basic health insurance is compulsory for everyone living in Switzerland, and you must arrange it yourself within three months of arriving – it is not deducted from your salary and no employer does it for you. What it covers is written into federal law, so every insurer offers exactly the same medical cover and must accept you regardless of your health.

That is why the table below is a price list, not a product comparison. A cheaper premium buys the same treatment from a different company. What you choose is the insurer, the deductible (CHF 300 to 2,500 for adults) and the model – whether you may go to any doctor or agree to contact a family doctor, an HMO practice or a telemedicine line first.

Why is the premium different in the next canton?

Because it reflects the costs actually incurred there. The cover is the same across the country – what differs are the health costs of the region: density of hospitals and doctors, number of treatments, price level. Premiums follow those costs because each canton carries its own.

This is why switching insurer within your canton is the one lever genuinely in your hands. The spread within a canton is usually wider than the gap to the neighbouring one – several hundred francs a year often separate the cheapest insurer from the most commonly chosen one, at identical cover.

By when must I cancel to switch for 2026?

Basic insurance runs to the end of the calendar year with one month's notice, so you cancel in the previous year: the letter had to arrive at your previous insurer by 30 November 2025 – sending it in time is not enough, so send it by registered post.

Changing only your deductible needs no change of insurer – but the same deadline applies. Work out the deductible with your own costs

Source and method

  • Premiums: Swiss Federal Office of Public Health (FOPH), tariff year 2026. Analysed on 05.08.2026.
  • Compared is the cheapest offer per canton for adults at a CHF 2,500 deductible including accident cover. The monthly redistribution of the environmental levy is already deducted – which is how the amount appears on your invoice.
  • The change compares each product with itself – same insurer, same model, same region – across all offers in the canton, not only the ones shown. The figure given is the median.
  • The Swiss figure is the mean of the cantonal medians, weighted by insured population (8,982,579 insured).
  • Sorting is strictly by premium, ascending. The order does not depend on whether Nexin works with an insurer.
  • The insurer's policy is always what counts.
