Before you compare: what you are actually choosing

Basic health insurance (Grundversicherung, KVG/LAMal) is compulsory for everyone living in Switzerland, and what it covers is written into federal law. Every insurer must offer exactly the same medical cover and must accept you regardless of your health. So a cheaper premium does not buy you less treatment – it buys the same treatment from a different company.

What you actually choose is three things: the insurer, the deductible (Franchise, CHF 300 to 2,500 for adults) and the model – whether you may go to any doctor, or agree to call a family doctor, an HMO practice or a telemedicine line first. Restricting your first point of contact is what makes the saver models cheaper.

Two things are not your choice: the canton and municipality you live in, which set the premium region, and your age group. Supplementary insurance (Zusatzversicherung) is a separate, voluntary product with its own rules – insurers may reject you there.

The most commonly chosen insurer here is CSS (28% of the insured). Its cheapest offer costs CHF 228.25 a month – CHF 312 more a year than the cheapest offer, for cover that is identical by law. In basic insurance market share measures inertia, not quality.

Children and young adults

Children are insured cheaply – from CHF 15.35 a month. That ends on 1 January following the 18th birthday: the same person then counts as a young adult and pays at least CHF 144.65, roughly 9.4 times as much and CHF 1,552 more a year. The jump comes with the change of age group, not with the birthday itself.

  • No compulsory deductible. For children the deductible is voluntary and the lowest tier is zero. Because the break-even point is low, zero is the cheaper choice for most children.
  • Half the co-payment cap. Above the deductible children pay at most CHF 350 a year themselves, against CHF 700 for adults. If several children of one family are insured with the same insurer, together they pay at most the cost-sharing of one adult – so at most CHF 300 deductible and CHF 700 co-payment (Art. 64 para. 4 KVG). With different insurers this cap does not apply.
  • Premium reductions for children are mandatory. Cantons must reduce the premiums of children in lower and middle income households by at least 80%, and those of young adults in education by at least 50% (Art. 65 para. 1bis KVG). You apply to your canton of residence, not to the insurer.
  • Register before the birth. Registering a child only after birth risks rejection or a lifelong exclusion in supplementary insurance. It does not affect basic insurance – there, insurers must accept every applicant.

By when must I cancel to switch for 2026?

Basic insurance runs to the end of the calendar year with one month's notice. To switch for the 2026 premium year you therefore had to cancel in the previous year: the letter had to arrive at your previous insurer by 30 November 2025 – sending it in time is not enough, so send it by registered post.

The cancellation only takes effect once the new insurer confirms cover, so never cancel before you have that confirmation. Supplementary insurance has its own, usually earlier deadlines – by the end of November it is normally too late for those.

There is a second date almost nobody uses: if you have the ordinary CHF 300 deductible and free choice of doctor, you may also switch at the end of June, with three months' notice – so by the end of March (Art. 7 KVG). With a saver model or a higher deductible that route is closed and the year end is your only option.
